WELCOME, IT'S GREAT TO SEE YOU ALL AND WELCOME TO THE H1N1 INFLUENZA
PREPAREDNESS SUMMIT.
I'M DR. HOWARD KOH, ASSISTANT SECRETARY FOR HEALTH FOR THE
DEPARTMENT OF HEALTH AND HUMAN SERVICES AND IT'S MY GREAT
PRIVILEGE TO WELCOME YOU AND TO SERVE AS YOUR MODERATOR
FOR TODAY'S EVENTS.
WE ARE ABSOLUTELY DELIGHTED TO SEE THIS TURN OUT AND TO SEE SO MANY
LEADERS HERE AT THE FEDERAL, STATE, TRIBAL, TERRITORIAL
AND LOCAL LEVELS AND WE KNOW THAT MANY OF YOU HAVE TRAVELED MANY
MILES TO COME HERE TODAY AND WE ARE VERY GRATEFUL TO YOU FOR
YOUR DEDICATION AND YOUR WILLINGNESS TO LEAD AND
SERVE IN THIS CRITICAL TIME IN OUR NATION'S HISTORY.
TODAY'S SUMMIT SERVES AS A CALL TO ACTION FOR ALL OF US TO WORK
IN PARTNERSHIP TO ENSURE THAT OUR COUNTRY IS PREPARED FOR INFLUENZA,
ESPECIALLY GIVEN THE EMERGENCE OF THE NOVEL H1N1 VIRUS.
WE HAVE A VERY AMBITIOUS AGENDA TODAY AND OUR GOALS ARE THE
FOLLOWING: WE WILL REVIEW LESSONS LEARNED FROM THE
SPRING H1N1 FLU OUTBREAK.
WE WILL DISCUSS BEST PRACTICES BY STATE AND LOCAL GOVERNMENTS.
WE WILL PROVIDE AN UPDATE ON THE EFFORT TO DEVELOP AND POTENTIALLY
ADMINISTER AN H1N1 VACCINE AND, VERY IMPORTANTLY,
WE WILL EXAMINE STRATEGIES FOR EFFECTIVELY COMMUNICATING WITH
STAKEHOLDERS AND THE PUBLIC.
SO TO BEGIN I'D LIKE TO INTRODUCE DR. RAYNARD KINGTON, ACTING
DIRECTOR OF THE NATIONAL INSTITUTES OF HEALTH WHO
WILL GIVE US OPENING REMARKS.
DR. KINGTON WAS APPOINTED DEPUTY DIRECTOR OF NIH IN FEBRUARY 2003
AND HAS SERVED AS ACTING NIH DIRECTOR SINCE OCTOBER 2008.
HE HAS PLAYED A KEY ROLE IN LEADERSHIP WITH RESPECT TO
POLICY DIRECTION AND COORDINATION OF ALL NIH BIOMEDICAL RESEARCH
AND RESEARCH TRAINING PROGRAMS.
PLEASE JOIN ME IN SAYING WELCOME TO DR. KINGTON.

[DR. KINGTON] GOOD MORNING.
SECRETARY SEBELIUS, SECRETARY DUNCAN, SECRETARY NAPOLITANO,
FELLOW COLLEAGUES AND DISTINGUISHED GUESTS,
IT'S MY DISTINCT HONOR TO WELCOME ALL OF YOU TODAY
TO NIH FOR TODAY'S H1N1 INFLUENZA PREPAREDNESS SUMMIT.
OUR SUBJECT TODAY IS BOTH TIMELY AND POTENTIALLY URGENT.
YET, DESPITE THE VERY CURRENT THREAT IT POSES, INFLUENZA
HAS A LONG AND FAIRLY WELL-DEFINED PAST.
AS IT TURNS OUT, THE HISTORIES OF INFLUENZA AND THE NATIONAL
INSTITUTES OF HEALTH HAVE CONVERGED WITH
THE EPIDEMIC'S RESURGENCES.
JUST LAST WEEK IN THE NEW ENGLAND JOURNAL OF MEDICINE OUR OWN
DR. ANTHONY FAUCI, WHO WILL ADDRESS YOU SHORTLY AND HIS
COLLEAGUES, DISCUSSED THE GENETIC EVIDENCE ILLUSTRATING THAT THE
PRESENT H1N1 VIRUS TRACES ITS ORIGINS BACK TO THE VIRUS THAT
CAUSED THE INFAMOUS INFLUENZA EPIDEMIC OF 1918 TO 1919.
WE ARE, THEREFORE, LIVING IN A PANDEMIC ERA THAT HAS LASTED
NEARLY A CENTURY AND OVER THE COURSE OF THAT CENTURY THE
NIH HAS TAKEN A LEADING ROLE IN SHAPING THE NATION'S RESPONSE
TO THE VIRUS' PERSISTENT RECURRENCES IN ITS MANY GUISES.
I'LL MENTION JUST THE FIRST OF THESE OCCASIONS.
AT THE END OF WORLD WAR I THE U.S. PUBLIC HEALTH SERVICE HELPED
COORDINATE THE COUNTRY'S EFFORTS TO CONTAIN THE INFLUENZA PANDEMIC
THAT WAS SWEEPING THE GLOBE.
THE PUBLIC HEALTH SERVICE OFFICERS COMPILED STATISTICS,
BOTH NATIONALLY AND INTERNATIONALLY,
TO PROVIDE A CLEAR PICTURE OF THE INFLUENZA'S SCOPE,
AND THE NATIONAL HEALTH SERVICES HYGIENIC LAB,
WHICH WAS THE PREDECESSOR TO THE NIH, PLAYED A KEY ROLE
IN ANALYZING THOSE STATISTICS AND DEVISING AND TESTING
INFLUENZA VACCINES.
IN THE PROCESS, THE DIRECTOR OF THE HYGIENIC LAB, DR. GEORGE
MCCOY, HELPED FORGE THE FIRST PROFESSIONAL STANDARDS FOR
VACCINE TRIALS IN AMERICA.
THOSE STANDARDS GUIDED VACCINE TRIAL DESIGN WELL INTO THE 1940S.
OVER THE PAST 90 YEARS, BIOMEDICAL RESEARCH HAS REVEALED MUCH ABOUT
INFLUENZA THAT OUR PREDECESSORS COULD NOT HAVE KNOWN.
THAT KNOWLEDGE WILL HELP US RESPOND QUICKLY AND
EFFICIENTLY TO ANY EPIDEMIC THREAT.
YOU'LL HEAR MORE ABOUT THE STATE OF OUR SCIENCE, IN PARTICULAR AS
IT RELATES TO THE CREATION OF VACCINES, LATER TODAY.
THIS IS, IN FACT, THE ROLE THAT CONGRESS ASKED NIH TO PLAY.
IT IS OUR MISSION TO SEEK SCIENTIFIC KNOWLEDGE THAT
WILL IMPROVE THE PUBLIC'S HEALTH.
BIOMEDICAL RESEARCH GIVES US POWERFUL TOOLS TO LESSEN THE
DAMAGE THAT INFECTIOUS DISEASES CAN INFLICT OR,
IDEALLY, TO PREVENT THEM ALTOGETHER.
YET, PANDEMICS ARE NOT JUST ABOUT RESEARCH ANYMORE THAN THEY ARE
JUST ABOUT MEDICINE OR EVEN PUBLIC HEALTH.
PANDEMICS AFFECT ALL OF SOCIETY.
CONSEQUENTLY, ALL PARTS OF SOCIETY MUST WORK TOGETHER
IF WE ARE TO SUCCEED IN OUR EFFORTS TO CONTROL EPIDEMICS.
LOCAL AND NATIONAL GOVERNMENT, BUSINESSES, SCHOOLS, COMMUNITIES,
EVEN FAMILIES MUST WORK TOGETHER WITH THE MEDICAL ESTABLISHMENT
TO PROVIDE A UNIFIED FRONT AGAINST INFLUENZA OR ANY
EPIDEMIC THREAT AND THAT IS WHY ALL OF US ARE HERE TODAY.
THIS SUMMIT PROVIDES US WITH THE OPPORTUNITY TO LEARN FROM EACH
OTHER, TO CREATE CENTRAL LINES OF COMMUNICATION AND DEVISE
STRATEGIES FOR SUCCESS.
AS WE MOVE TOGETHER TO CONFRONT FUTURE CHALLENGES FROM INFLUENZA
OR OTHER INFECTIONS, WE CAN BE CONFIDENT THAT WE HAVE LEARNED
OUR LESSONS FROM THE PAST AND THAT WE ARE BRINGING OUR BEST KNOWLEDGE
TO OUR PRESENT EFFORTS.
WE WILL, THEREFORE, BE EQUIPPED TO MEET ANY PANDEMIC CHALLENGE
WITH COOPERATIVE AND WELL-INFORMED RESOLVE.
MY PERSONAL KNOWLEDGE OF THE TREMENDOUS WORK BEING DONE
HERE AT NIH, AT OUR SISTER AGENCIES, PARTICULARLY
THE CDC AND FDA, AT STATE AND LOCAL HEALTH DEPARTMENTS AND
AT UNIVERSITIES AND IN COMPANIES ACROSS THE COUNTRY, GIVES ME
CONFIDENCE THAT WE WILL, INDEED, SUCCEED.
BY THE END OF TODAY'S SESSION I BELIEVE YOU TOO WILL SHARE
MY CONFIDENCE IN OUR COMBINED ABILITY TO, IF NECESSARY,
TAKE ON INFLUENZA AND TAKE AWAY ITS FORCE.
SO AGAIN, AT THIS CONVERGENCE OF TIMELY OCCASION AND HISTORIC
LOCATION, IT'S MY PRIVILEGE TO WELCOME YOU TO THIS H1N1
INFLUENZA PREPAREDNESS SUMMIT.

MR. BRENNAN HAS SERVED AS ASSISTANT TO THE PRESIDENT
FOR HOMELAND SECURITY AND COUNTERTERRORISM SINCE
JANUARY 20, 2009.
HE ALSO SERVES AS HOMELAND SECURITY ADVISOR AND DEPUTY
NATIONAL SECURITY ADVISER FOR COUNTERTERRORISM.
MR. BRENNAN SERVES AS THE PRINCIPAL WHITE HOUSE STAFF
MEMBER RESPONSIBLE FOR THE FORMULATION AND COORDINATION
AND IMPLEMENTATION OF POLICIES RELATED TO HOMELAND SECURITY
AND COUNTERTERRORISM.
MR. BRENNAN HAS A DISTINGUISHED 25 YEAR CAREER WITH THE CENTRAL
INTELLIGENCE AGENCY, SERVING IN A VARIETY OF SENIOR POSITIONS
THROUGHOUT THE INTELLIGENCE COMMUNITY.
HIS LAST ASSIGNMENT -- WHOA -- YOU MUST BE VERY IMPORTANT.

HIS LAST ASSIGNMENT WAS INTERIM DIRECTOR OF THE
NATIONAL COUNTERTERRORISM CENTER.
HE WAS ALSO THE FOUNDING DIRECTOR OF THE NATIONAL COUNTERTERRORISM
CENTER'S PREDECESSOR ORGANIZATION, THE TERRORISM
THREAT INTEGRATION CENTER.

[JOHN BRENNAN] THANK YOU VERY MUCH, DR. KINGTON, AND GOOD MORNING.
I WOULD LIKE TO EXTEND A WARM WELCOME TO EVERY ONE ON BEHALF
OF PRESIDENT OBAMA, WHO YOU WILL BE HEARING FROM SHORTLY.
I'D ALSO LIKE TO EXTEND A STRONG NOTE OF APPRECIATION TO DR. FAUCI
AT NIH AND EVERYBODY WHO MADE THIS CONFERENCE POSSIBLE TODAY.
IN ADDITION, I'D LIKE TO SAY THANK YOU TO SECRETARY SEBELIUS,
NAPOLITANO AND DUNCAN FOR HOSTING THIS IMPORTANT DAY-LONG EVENT.
WE ARE HERE TODAY BECAUSE H1N1 POSES A SERIOUS THREAT TO
PUBLIC HEALTH, EVEN A DEADLY THREAT.
WE HAVE WATCHED THIS INFLUENZA OVER THE LAST SEVERAL MONTHS
SPREAD IN OUR COUNTRY AND THROUGHOUT THE WORLD, AND
IT WILL CONTINUE TO SPREAD.
THE U.S. GOVERNMENT HAS COMMITTED TO DOING EVERYTHING POSSIBLE TO
MINIMIZE THE IMPACT OF H1N1.
PRESIDENT OBAMA HAS GIVEN US CLEAR DIRECTION: SPARE NO
EFFORT WHATSOEVER TO PROTECT THE HEALTH AND SAFETY
OF THE AMERICAN PEOPLE.
HE HAS OBTAINED SIGNIFICANT FUNDING AND HE HAS MOBILIZED
THE RESOURCES OF THE GOVERNMENT TO MAKE SURE THAT WE'RE ABLE TO
ADDRESS THIS GROWING PUBLIC HEALTH THREAT.
I WOULD LIKE TO JUST MAKE THREE BRIEF POINTS THIS MORNING.
FIRST, ALTHOUGH WE HAVE MUCH KNOWLEDGE ABOUT PANDEMIC
INFLUENZAS, THERE IS STILL A LOT THAT IS UNKNOWN AND WE DON'T
HAVE ALL THE ANSWERS.
THIS IS A VERY DYNAMIC SITUATION AND OUR KNOWLEDGE OF THIS
INFLUENZA WILL GROW OVER THE COMING MONTHS, AS WELL AS
OUR ABILITY TO TELL YOU EXACTLY WHAT THE GOVERNMENT IS ABLE TO
DO IN ORDER TO RESPOND TO IT.
WE DON'T HAVE ALL THE ANSWERS BUT WE ARE COMMITTED TO MAKING SURE
THAT WE ENGAGE IN OPEN AND HONEST COMMUNICATION WITH THE ENTIRE
AMERICAN COMMUNITY AND PARTICULARLY WITH THOSE
RESPONSIBLE FOR ENSURING THE PROTECTION OF PUBLIC HEALTH.
SECONDLY, TIME IS OF THE ESSENCE.
OVER THE LAST SEVERAL MONTHS WE HAVE WORKED HARD TO MITIGATE
AND TO MINIMIZE THE IMPACT OF H1N1 BUT WE ARE FACING A FALL SEASON
WHERE WE ARE CERTAIN THAT H1N1 IS GOING TO AFFECT OUR COUNTRY.
WE NEED TO MAKE SURE WE DO EVERYTHING POSSIBLE TO PLAN
AND PREPARE NOW BECAUSE WE KNOW THAT WE ARE GOING TO BE FACING
THIS CHALLENGE OVER THE COMING MONTHS.
AND THIRD, THIS IS CLEARLY A TEAM EFFORT.
THE FEDERAL GOVERNMENT MUST WORK WITH STATE AND LOCAL GOVERNMENTS,
WITH PUBLIC HEALTH OFFICIALS, WITH SCHOOL ADMINISTRATORS,
WITH COMMUNITY LEADERS, WITH FAMILIES AND WITH INDIVIDUALS
AND PRESIDENT OBAMA IS COMMITTED TO MAKING SURE THAT WE ACTUALLY
DO ENGAGE IN A VERY STRONG AND MAYBE UNPRECEDENTED TEAMWORK
[BREAK IN AUDIO]
THIS IS SOMETHING THAT HE HAS TOLD US TO DO AND WE
ARE COMMITTED TO DOING IT.
WE HAVE THE OPPORTUNITY TO MINIMIZE THE IMPACT OF H1N1.
WE NEED TO MAKE SURE WE DO EVERYTHING POSSIBLE NOW IN
ORDER TO ACHIEVE THAT GOAL AND DIRECTIVE.
PRESIDENT OBAMA HAS TOLD US THAT, ALTHOUGH THERE ARE
MANY UNCERTAINTIES HERE, ONE THING IS CERTAIN: WE NEED TO MAKE SURE
WE DO WHAT WE CAN NOW.
AGAIN, ON BEHALF OF PRESIDENT OBAMA, I'D LIKE TO THANK YOU
FOR BEING HERE TODAY AND WE LOOK FORWARD TO ENGAGING WITH YOU AND

DR. TOM FRIEDMAN, WHO IS THE NEW DIRECTOR OF THE CENTERS FOR
DISEASE CONTROL AND PREVENTION AND HE'LL BE PROVIDING A SITUATIONAL
UPDATE ON THE 2009 H1N1 INFLUENZA VIRUS.
DR. FRIEDMAN BECAME CDC DIRECTOR ON JUNE 8, AND IN DOING SO,
REJOINS AN AGENCY WHERE HE PREVIOUSLY WORKED FROM 1990
UNTIL 2002 AND PRIOR TO HIS RECENT APPOINTMENT HE SERVED
MOST ABLY AS COMMISSIONER OF THE NEW YORK CITY DEPARTMENT
OF HEALTH AND MENTAL HYGIENE, ONE OF THE WORLD'S OLDEST AND LARGEST
PUBLIC HEALTH AGENCIES.
DR. FRIEDMAN IS A PHYSICIAN WHO HAS TRAINED IN INTERNAL MEDICINE
AND INFECTIOUS DISEASES, PUBLIC HEALTH AND EPIDEMIOLOGY AND HE
HAS CONSIDERABLE EXPERTISE IN INFECTIOUS DISEASE CONTROL,
AS YOU WILL SOON SEE.
SO PLEASE WELCOME DR. FRIEDMAN.

[TOM FRIEDMAN] OUR GOALS AT THIS TIME ARE FAIRLY STRAIGHTFORWARD: REDUCE ILLNESS
AND DEATH AND MINIMIZE SOCIAL DISRUPTION.
THE OUTBREAK IN THE PAST YEAR HAS BEEN CHARACTERIZED BY A GREAT DEAL
OF UNPREDICTABILITY AND, IN FACT, INFLUENZA MAY BE THE
MOST UNPREDICTABLE OF ALL INFECTIOUS DISEASES.
THIS OUTBREAK OCCURRED LATE IN THE SEASON.
THERE WAS REMARKABLE HETEROGENEITY ACROSS THE U.S.
IT AFFECTED YOUNG PEOPLE DISPROPORTIONATELY.
IT CAUSED WIDESPREAD ILLNESS, INCLUDING SOME SEVERE
ILLNESS AND DEATHS.
IT WAS SOCIALLY DISRUPTIVE, ESPECIALLY FOR SCHOOLS AND
TENS OF THOUSANDS OF HEALTH WORKERS AND OTHERS HAVE BEEN
RESPONDING AND CONTINUE TO RESPOND WORLDWIDE.
THIS IS A DEMONSTRATION OF THE VARIABLE IMPACT IN DIFFERENT
PARTS OF THE U.S., ALTHOUGH THE DIAGNOSIS IS A LITTLE DIFFERENT
AND TESTING IS A LITTLE DIFFERENT.
THE POINT HERE IS THAT THERE IS ACTUALLY A 100-FOLD DIFFERENCE
IN THE LEVEL OF DOCUMENTED INFECTION IN DIFFERENT
STATES IN THE U.S. AND EVEN WITHIN STATES AND EVEN WITHIN CITIES
THERE IS A GREAT DEAL OF HETEROGENEITY SO WHAT ONE
COMMUNITY EXPERIENCES IS NOT NECESSARILY WHAT ANOTHER
COMMUNITY, EVEN NEARBY, IS LIKELY TO EXPERIENCE.
PANDEMIC INFLUENZA IS NOW CAUSING NEARLY ALL TYPEABLE
INFLUENZA IN THE U.S.
WHAT YOU SEE IN THE DARK BLUE BARS IS THE NUMBER OF 2009
H1N1 PANDEMIC FLU AND IN THE EARLY PART OF THIS SEASON [INAUDIBLE]
IN THE EARLY PART OF THIS SEASON THIS WAS PRIMARILY A SEASON THAT
WAS CHARACTERIZED BY INFLUENZA B; THERE'S NON-SUBTYPES -- YOU CAN
KIND OF IGNORE THE YELLOW LINES AND THE OLD A1 AND THEN HERE WE
SEE THE INCREASING A1 AND IN THE LINE HERE YOU SEE THE PROPORTION
OF ALL THAT ARE POSITIVE, AND SO WHAT THIS IS BASICALLY SAYING
IS THAT AT THIS TIME IN THE U.S. WHERE THERE IS STILL
A LOT OF H1N1, IT'S MOSTLY THE PANDEMIC STRAIN.
WHAT THAT MEANS FOR THE FALL: NOBODY KNOWS.
IT'S GOING TO BE VERY IMPORTANT TO MONITOR
WHAT HAPPENS AS TIME GOES ON.
TEENS AND YOUNG ADULTS WERE DISPROPORTIONATELY AFFECTED,
AND WE HAD RELATIVELY FEW CASES AMONG THE ELDERLY SO IF YOU LOOK
JUST AT THOSE OVER 65, A STRIKING DIFFERENCE AMONG HOSPITALIZED
PATIENTS BETWEEN SEASONAL FLU, TAKING A TYPICAL YEAR,
AND THE H1N1 FLU EPIDEMIOLOGY.
ALSO ALONG WITH THE RELATIVELY FEWER CASES AMONG THE ELDERLY,
MORE CASES AMONG SCHOOL AGE CHILDREN, FIVE TO 17.
A FEW MORE CASES ALSO AMONG YOUNG ADULTS -- I HAVE TO HAVE CASES
SOMEWHERE: NOT A LOT MORE PROPORTIONATELY AMONG
CHILDREN AND, AGAIN, MANY MORE AMONG THE 50 TO 64S AS WELL.
SO A VERY DIFFERENT PATTERN IN THIS OUTBREAK FROM A TYPICAL
SEASON WHERE MANY OF OUR HOSPITALIZED CASES WOULD
BE OVER 65 AND MANY MORE OVER 50.
THESE TWO SLICES WOULD GIVE YOU 60% OR MORE TO REALLY A RELATIVELY
SMALL PROPORTION IN THE CURRENT OUTBREAK.
AMONG PATIENTS HOSPITALIZED FOR 2009 H1N1, AN OVERWHELMING
PROPORTION, MORE THAN TWO THIRDS OR THREE QUARTERS HAVE ONE OR
MORE UNDERLYING HEALTH CONDITIONS, AND WHAT THOSE UNDERLYING HEALTH
CONDITIONS ARE, ARE IMPORTANT BECAUSE THEY IDENTIFY GROUPS
WHICH ARE PARTICULARLY AT RISK IN THE FUTURE WHICH NEED, IF AND WHEN
H1N1 RETURNS, WHICH NEED TO BE PROTECTED, WHETHER THAT'S BY
PROMPT TREATMENT TO REDUCE THE RISK OF PROGRESSION TO SEVERE
ILLNESS OR DEATH OR BY VACCINATION, WHEN THAT
BECOMES AVAILABLE, IF AND WHEN THAT BECOMES AVAILABLE.
ASTHMA IS THE LEADING SINGLE UNDERLYING DIAGNOSIS, PRESENT
IN NEARLY A THIRD OF HOSPITALIZED PATIENTS.
THE LIGHT BLUE BAR IS HOSPITALIZED PATIENTS.
THE GREEN BAR IS THE GENERAL POPULATION, SO YOU SEE THAT
PEOPLE WITH ASTHMA WERE MUCH MORE LIKELY TO BE HOSPITALIZED THAN
IN THE GENERAL POPULATION AND ASTHMA IS NOT UNCOMMON
IN THE GENERAL POPULATION.
PEOPLE WITH CHRONIC LUNG DISEASE, PEOPLE WITH DIABETES, CHRONIC
HEART DISEASE, PEOPLE WITH MORBID OBESITY; THAT'S A BODY MASS INDEX
OF 40 OR MORE AND THEN PARTICULARLY AMONG CHILDREN,
NEUROCOGNITIVE AND NEUROMUSCULAR DISORDERS AND PREGNANT WOMEN.
SO THESE ARE GROUPS WHICH WE KNOW ARE AT HIGHER RISK OF SEVERE
DISEASE AND HIGHER PRIORITY FOR PROTECTION GOING FORWARD.
THE ESTIMATED IMPACT OF INFLUENZA PANDEMICS IS LARGE.

ALTHOUGH INFECTION RATES HAVE DECLINED IN MANY PARTS OF THE
U.S., OUTBREAKS ARE CONTINUING AND THERE HAVE BEEN MORE THAN
50 OUTBREAKS IN SUMMER CAMPS.
SOME COMMUNITIES CONTINUE TO HAVE HIGH RATES OF INFECTION.
IN THE SOUTHERN HEMISPHERE WE HAVE SEEN SUBSTANTIAL DISEASE
IN ARGENTINA, CHILE, AUSTRALIA; REPORTED CASES FROM MUCH OF ASIA
AND AFRICA; AND IN SOME AREAS CO-CIRCULATION WITH SEASONAL
INFLUENZA; SO DIFFERENT PATTERNS OF INFECTION IN DIFFERENT PARTS
OF THE COUNTRY, DIFFERENT PARTS OF THE WORLD AND STRAIN ON HEALTH
CARE SYSTEMS IN SOME LOCALITIES.
ANTIVIRAL RESISTANCE TO OSELTAMIVIR, TAMIFLU,
HAS BEEN DETECTED IN AT LEAST THREE DIFFERENT
PARTS OF THE WORLD.
NONE OF THOSE DETECTIONS HAVE REPRESENTED A WIDE SCALE CHANGE
IN THE RESISTANCE PATTERN THAT WOULD RESULT IN ANY CHANGE
IN OUR TREATMENT RECOMMENDATIONS BUT IT'S CLEARLY SOMETHING THAT
NEEDS TO BE WATCHED CAREFULLY.
IN RED ARE THE COUNTRIES WHICH HAVE DOCUMENTED
MORE THAN A THOUSAND CASES.
YOU CAN SEE WIDESPREAD AROUND THE WORLD IN A SLIGHTLY LIGHTER COLOR,
DOCUMENTED HUNDREDS OF CASES AND THIS, OBVIOUSLY, IT'S A RESULT
OF DIAGNOSTIC AND LABORATORY CAPACITY AS WELL: DOZENS OF
CASES, SOME CASES, SO YOU CAN SEE NOW THAT THE PANDEMIC IS TRULY
A PANDEMIC AND AFFECTING VIRTUALLY ALL AREAS OF THE WORLD.
THOSE AREAS WHICH HAVE NOT YET DOCUMENTED A CASE MAY
BE A QUESTION OF DIAGNOSTIC OR TESTING OR MAY JUST BE THAT
THEIR TIME HAS NOT QUITE COME YET.
EVENTS COULD CHANGE.
THE PROPORTION OF SEVERE DISEASE COULD CHANGE.
RIGHT NOW THE LEVEL OF SEVERITY APPEARS TO BE SIMILAR OR NO
WORSE THAN THAT OF SEASONAL FLU.
IT'S IMPORTANT TO RECOGNIZE, THOUGH, THAT SEASONAL FLU
KILLS 36,000 PEOPLE EVERY YEAR IN THIS COUNTRY SO THAT'S NOT
TERRIBLY REASSURING TO SAY IT'S NOT WORSE THAN SEASONAL FLU.
THE TRANSMISSIBILITY OF H1N1 IS SOMETHING THAT WE'RE STILL TRYING
TO UNDERSTAND FROM A BASIC SCIENCE AND EPIDEMIOLOGY PERSPECTIVE, BUT
CLEARLY, IT HAS CAUSED LARGE EXPLOSIVE OUTBREAKS IN SOME
SCHOOLS AND REALLY HAS A DIFFERENT PATTERN IN TERMS OF INCREASING
AGE, DECREASING RISK OF INFECTION AND SEVERE DISEASE.
ANTIVIRAL RESISTANCE PATTERNS WILL BE VERY IMPORTANT TO MONITOR OVER
TIME BECAUSE THOSE WILL HAVE TREATMENT IMPLICATIONS AND
WE ARE ALL EAGERLY AWAITING THE DEVELOPMENT AND TESTING
OF A VACCINE CURRENTLY UNDER WAY TO LOOK AT THE EFFECTIVENESS,
SAFETY AND MATCH WITH THE ANTIGENS OF THE VIRUS.
AND WE'LL NEED TO ADJUST OUR PLANS, BASED ON DATA.
WHAT CAN WE DO AT THIS TIME INCLUDES, FIRST, SURVEILLANCE:
TRACKING, MONITORING, SEEING WHAT'S HAPPENING IN OUR COUNTRY
AND GLOBALLY BECAUSE ALL OF THIS WILL BE VERY IMPORTANT TO US IN
FORMING OUR OWN RESPONSE.
PLANNING AT ALL LEVELS AS APPROPRIATE AT EACH LEVEL'S
ROLE IN PREVENTION AND RESPONSE: THE HEALTH CARE SYSTEM, THE
DIAGNOSIS, TREATMENT, SURGE AND VACCINATION AMONG OTHER ISSUES:
SCHOOLS, CHILD CARES, JAILS, HOMELESS SHELTERS, OTHER
CONGREGATE SETTINGS PREPARING FOR WHAT MIGHT HAPPEN IN THE FALL:
PUBLIC HEALTH ADDRESSING SURVEILLANCE AND TESTING
RESPONSE, GUIDANCE, VACCINATION AND TREATMENT.
VACCINATION: UNDERSTANDING THAT THERE MAY BE GROUPS THAT WE'RE
NOT TRADITIONALLY REACHING OUT TO FOR FLU VACCINATION WHICH ARE HIGH
PRIORITY THIS YEAR OR WHICH DON'T DO A PARTICULARLY GOOD JOB WITH
SEASONAL FLU VACCINATION, WHICH WE NEED TO REACH
AT HIGH NUMBERS THIS YEAR.
EFFECTIVE COMMUNICATION IS ESSENTIAL.
EFFECTIVE COMMUNICATION DEPENDS ON GOOD INFORMATION.
I CAN TELL YOU THAT FROM MY VANTAGE POINT OF HAVING BEEN
HEALTH COMMISSIONER IN NEW YORK CITY WHILE WE HAD A VERY LARGE
OUTBREAK OF H1N1 WITH HUNDREDS OF THOUSANDS OF CASES, MORE
THAN 50 SCHOOLS THAT WE HAD TO CLOSE FOR A VARIETY OF REASONS,
THERE'S KIND OF A FOG OF WAR NATURE OF THE INFORMATION.
"OH, THERE'RE CLUSTERS HERE.
THERE'RE DEATHS HERE.
THERE'RE HOSPITALS ON DIVERSIONS HERE," AND BEING ABLE TO MANAGE
THAT INFORMATION IN THE KIND OF FISH BOWL ENVIRONMENT THAT WE
OFTEN WORK IN TODAY IS ENORMOUSLY CHALLENGING AND SOMETHING THAT
WE'LL BE DISCUSSING HOW IT CAN BE DEALT WITH EFFECTIVELY
GOING FORWARD.
DARWIN SAID "IT'S NOT THE STRONGEST OF THE SPECIES
THAT SURVIVES NOR THE MOST INTELLIGENT.
IT'S THE ONE THAT'S MOST ADAPTABLE TO CHANGE."
AND I THINK AS WE LOOK AT THE CHALLENGE OF H1N1, WE NEED TO
LOOK AT ADAPTING AS WE UNDERSTAND MORE ABOUT
THE EPIDEMIOLOGY OF THE VIRUS: AS WE UNDERSTAND MORE ABOUT
THE PATTERNS, ADJUSTING OUR PLANS TO FIT THE SITUATION SO THAT OUR
RESPONSE IS APPROPRIATE TO THE LEVEL OF ILLNESS AND WE CAN GO
BACK TO THOSE TWO FUNDAMENTAL GOALS OF REDUCING ILLNESS AND
DEATH AND MINIMIZING SOCIAL DISRUPTION.

AS YOU'VE HEARD, PREPARING FOR PANDEMIC FLU IS A VERY IMPORTANT
CHALLENGE BUT IT'S PART OF A MUCH LARGER CHALLENGE OF PREPARING FOR
EMERGING AND REEMERGING INFECTIOUS DISEASES IN GENERAL, AND THIS IS
A PERPETUAL CHALLENGE AND SOMETHING THAT WE HERE
AT NIH, TOGETHER WITH OUR COLLEAGUES IN CDC, HAVE
FOCUSED ON FOR DECADES.
WE HAVE SEEN MANY EMERGING DISEASES, EVEN IN RECENT
YEARS, FROM HIV/AIDS TO EBOLA TO WEST NILE TO SARS AND NOW
A NOVEL H1N1 INFLUENZA VIRUS.
WITH THIS INFLUENZA VIRUS, WE ARE FORTUNATE TO HAVE SOMETHING OF A
HEAD START, NOT ONLY WITH OUR RECENT H5N1 AVIAN INFLUENZA
PREPAREDNESS EFFORT BUT ALSO WITH OUR LONG HISTORY OF STUDYING
SEASONAL INFLUENZA, AS YOU'VE JUST HEARD FROM DR. FRIEDMAN,
WHICH, AS WE ALL KNOW, IS A SERIOUS HEALTH PROBLEM YEAR
IN AND YEAR OUT.
WITH THE EMERGENCE OF THE 2009 H1N1 INFLUENZA VIRUS, WE HAVE
SEEN A WELL COORDINATED RESPONSE AND PLANNING EFFORT ACROSS HHS
THAT INCLUDES CDC, NIH, FDA, BARDA AND OTHER COMPONENTS
OF THE DEPARTMENT.
WE AT NIH ARE PLEASED TO BE ABLE TO INTERACT WITH LEADERS FROM
OTHER FEDERAL AGENCIES AND WITH STATE AND LOCAL OFFICIALS AS
WE WORK TO BEST PREPARE THE NATION FOR A POTENTIALLY MORE SEVERE
OUTBREAK OF H1N1 INFLUENZA.
EACH OF US REPRESENTED HERE TODAY HAS A KEY ROLE TO PLAY.
OUR JOB AT NIH IS CONDUCTING AND SUPPORTING THE BASIC RESEARCH
THAT LEADS TO A BETTER UNDERSTANDING OF THIS
VIRUS AND HOW IT CAUSES DISEASE.
BUILDING ON CDC'S EXTRAORDINARY EARLY WORK IN TRACKING, ISOLATING
AND CHARACTERIZING THE VIRUS, NIH AND OUR GRANTEES ARE CONDUCTING
MOLECULAR ANALYSES, STUDIES OF THE PATHOGENESIS, TRANSMISSIBILITY,
EVOLUTION AND VIRULENCE OF THE H1N1 VIRUS AND STUDIES OF HOW
THE IMMUNE SYSTEM RESPONDS.
THIS INFORMATION WILL INFORM THE DESIGN AND DEVELOPMENT OF WHAT
WE CALL COUNTERMEASURES, INCLUDING DIAGNOSTICS,
NEW DRUGS AND ESPECIALLY A VACCINE; SO NOW A VERY BRIEF
WORD ON VACCINES.
FOR SEVERAL DECADES NIAID HAS BEEN INTENSIVELY INVOLVED IN
THE SUCCESSFUL DEVELOPMENT AND CLINICAL TESTING OF VACCINES
AGAINST MANY PATHOGENS THAT THREATEN THE HEALTH OF OUR
NATION AND THE WORLD.
AMONG THESE, AMONG MANY, HAVE BEEN SEASONAL INFLUENZA AND THE RECENT
H5N1 AVIAN INFLUENZA VACCINES.
WE LOOK FORWARD TO THE CHALLENGES OF BEING PART OF THE TEAM THAT
DEVELOPS A VACCINE FOR THE 2009 H1N1 INFLUENZA.
INDEED, WITH THE EMERGENCE OF THIS NOVEL INFLUENZA VIRUS,
WE HAVE SEEN A COLLABORATIVE AND ORDERLY PROCESS OF VACCINE
DEVELOPMENT THAT IS PROCEEDING IN A STEPWISE FASHION.
AFTER THE ISOLATION AND CHARACTERIZATION OF THE
VIRUS, THE CDC DISTRIBUTED SEED VIRUSES FOR THE DEVELOPMENT OF
PILOT LOTS OF VACCINES FOR TESTING IN CLINICAL TRIALS.
NIH WILL USE OUR LONG-STANDING VACCINE CLINICAL TRIALS
INFRASTRUCTURE, NOTABLY OUR NATIONWIDE NETWORK OF VACCINE
AND TREATMENT EVALUATION UNITS, TO QUICKLY EVALUATE PILOT LOTS
OF VACCINE CANDIDATES TO DETERMINE WHETHER THE VACCINE IS SAFE,
TO DETERMINE ITS ABILITY TO PRODUCE A PROTECTIVE IMMUNE
RESPONSE AND WHAT IS THE APPROPRIATE DOSE
AND NUMBER OF DOSES.
WE HOPE TO HELP EVALUATE THE FIRST CANDIDATE VACCINE
STARTING IN EARLY AUGUST.
THIS INFORMATION WILL BE CRITICAL TO THE DECISION ABOUT HOW AND IF
TO IMPLEMENT AN H1N1 VACCINATION PROGRAM THIS FALL.

OF THE WORK THAT HAS BEEN DONE SINCE I GOT HERE BUT, CERTAINLY,
THE WORK THAT HAS BEEN DONE OVER THE YEARS TO PREPARE FOR THIS DAY.
NOW THE HEADLINES AND TV CHATTER MAY HAVE NOT BEEN
COVERING THE FLU VIRUS.
THE VIRUS HASN'T GONE AWAY, AS TOM FRIEDMAN JUST LET US KNOW,
AND WE HAVE NOT LET UP.
WE'RE DOING EVERYTHING POSSIBLE TO MONITOR THE VIRUS AND PROTECT THE
AMERICAN PEOPLE: WORKING WITH CONGRESS, GOVERNORS, MAYORS,

NEW VIRUS AND TODAY'S SUMMIT IS A CRITICAL PART OF THOSE EFFORTS.
THE SUMMIT IS NOT ABOUT RAISING ALARMS OR STOKING FEARS.
IT'S ABOUT BEING PREPARED.
THIS IS A SERIOUS VIRUS, CAPABLE OF CAUSING SERIOUS DISEASE
AND EVEN DEATH.
IT'S TIME TO BE VIGILANT.
WE HAVE TO AVOID COMPLACENCY AND WE MUST BE PREPARED TO DEAL WITH
WHATEVER THE FALL BRINGS.
THE PRESIDENT HAS ANNOUNCED A NATIONAL FRAMEWORK FOR RESPONSE
TO THE 2009 H1N1 VIRUS AND THE RESPONSE IS BASED ON REALLY

VACCINATION CAMPAIGNS.
NINETY MILLION DOLLARS WILL HELP HOSPITALS PREPARE FOR THE SURGE
OF PATIENTS THEY WILL TREAT IF AN OUTBREAK IMPACTS THEIR COMMUNITY.

OF THIS OUTBREAK, THAT THE POTENTIAL FOR A SIGNIFICANT
OUTBREAK IN THE FALL IS LOOMING.
AND KATHLEEN, JANET, JOHN BRENNAN, ARNE DUNCAN AND OUR ENTIRE TEAM
HAVE TRIED TO ENGAGE IN THE MOST RIGOROUS PLANNING EXERCISES TO
MAKE SURE THAT ANYTHING THAT MAY OCCUR IN THE FALL WE ARE PREPARED
FOR AND SO I WON'T GO THROUGH THE DETAILS OF THIS.
I'M SURE THAT KATHLEEN AND JANET AND OTHERS HAVE LAID OUT WHAT
THE POTENTIAL CONSEQUENCES ARE OF A RENEWED OUTBREAK OF H1N1.
WE WANT TO MAKE SURE THAT WE ARE NOT PROMOTING PANIC BUT WE ARE
PROMOTING VIGILANCE AND PREPARATION AND THE MOST
IMPORTANT THING FOR US TO DO IN THIS PROCESS IS TO MAKE SURE
THAT STATE AND LOCAL OFFICIALS PREPARE NOW TO IMPLEMENT A
VACCINATION PROGRAM IN THE FALL BUT ALSO THAT THEY ARE WORKING
ON AN OVERALL PUBLIC COMMUNICATIONS CAMPAIGN
WITH THE WHITE HOUSE AND THE POSSIBILITIES THAT WE MAY
NEED TO BE DEALING WITH SCHOOLS THAT ARE SEEING SIGNIFICANT
OUTBREAKS OF H1N1.
AND, YOU KNOW, WE'VE LOOKED AT PAST CASES OF THIS BEING PROPERLY
HANDLED AND SITUATIONS LIKE THIS BEING IMPROPERLY HANDLED, AND ONE
OF THE MOST IMPORTANT DIFFERENCES IS WHERE IT'S WELL HANDLED, STATE
AND LOCAL OFFICIALS HAVE COMPLETE OWNERSHIP OVER THIS ISSUE.
THEY ARE PROVIDING GOOD IDEAS TO THE FEDERAL GOVERNMENT.
THEY ARE CRITICAL LINKS TO INFORM US: WHAT'S WORKING AND WHAT'S NOT.
AND SO I'M JUST VERY GRATEFUL THAT ALL OF YOU ARE TAKING

ONE POINT I'D LIKE TO EMPHASIZE FOR ALL OF US IS THAT THE WORK
WE DO NOW ON H1N1 IS NOT WASTED BECAUSE IT HAS APPLICATION IN
OTHER SCENARIOS AS WELL, SO EVEN THOUGH WE CANNOT AND SCIENCE
CANNOT YET PREDICT WHAT KIND OF FLU OR FLU EPIDEMIC WE ARE GOING
TO EXPERIENCE AND HOW SERIOUS ITS LETHALALITY WILL BE, WHETHER WE'RE
IN A 1918 SITUATION OR A 1968 SITUATION, THAT
REMAINS TO BE SEEN BUT WHAT WE DO KNOW, HOWEVER, IS THAT THE
PREPARATIONS WE DO NOW WOULD APPLY TO MANY OTHER SCENARIOS AS WELL
SO THE TIME, THE EFFORTS AND THE MONIES ARE PUT TO GOOD USE.
NOW, WHAT DOES HOMELAND SECURITY HAVE TO DO WITH THE FLU?
THE PLAIN FACT OF THE MATTER IS THAT WE'VE GOT A LOT.
HERE'S THE WAY IT'S SET UP IS UNDER WHAT'S CALLED HSPD5 THE
DIRECTOR OF HOMELAND, THE SECRETARY OF HOMELAND
SECURITY IS WHAT'S CALLED THE PRINCIPAL FEDERAL OFFICIAL.
WHAT DOES THAT MEAN?
WELL, WHAT THAT MEANS IS THAT WE KNOW HHS HAS TO LEAD ON ALL THE
PUBLIC HEALTH AND SCIENCE ASPECTS OF A FLU EPIDEMIC AND THAT'S WHY
IT'S SO IMPORTANT THAT WE WORK OUR ISSUES THROUGH IN TERMS OF
A VACCINE, VACCINE DISTRIBUTION, ANALYSIS OF THE KIND OF FLU THAT
WE'RE SEEING AND THE LIKE, BUT IF THERE TRULY IS A PANDEMIC
OF SERIOUS PROPORTION IT AFFECTS MANY, MANY OTHER THINGS.
IT AFFECTS SCHOOLS.
IT AFFECTS THE WORKPLACE.
IT AFFECTS OUR ABILITY TO PROVIDE NATIONAL SECURITY
IN A VARIETY OF WAYS.
IT AFFECTS CONTINUATION OF GOVERNMENT AND CONTINUATION
OF OPERATIONS WITHIN THE PRIVATE SECTOR; AND SO THE PLANNING TO BE
ABLE TO DO THAT UNDER A SCENARIO WHERE YOU COULD BE EXPERIENCING
HIGH RATES OF ABSENTEEISM AND WHERE ASSETS THAT YOU THINK
ARE AVAILABLE ARE NOT AVAILABLE ARE THE VERY ISSUES THAT THE
DEPARTMENT OF HOMELAND SECURITY IS CHARGED WITH COORDINATING.
SO THAT'S WHY I'M SO PLEASED TO BE HERE WITH MY COLLEAGUES TODAY.
WE'VE WORKED TOGETHER BEFORE.
WE'RE WORKING THIS PROBLEM AND THAT'S WHAT I CALL IT.

FIRST, WE KNOW THAT A FLU PANDEMIC CROSSES ALL SECTORS.

SO WE NEED TO MAKE SURE THAT PLANS ARE IN PLACE AND, HOPEFULLY, HAVE
BEEN ACTUALLY EXERCISED NOW SO THAT IF THERE ARE DIFFERENT
RATES OF ABSENTEEISM, OPERATIONS CONTINUE.
WELL, WHAT DOES THAT MEAN?
THAT MEANS YOU REALLY HAVE TO LOOK AT, FOR EXAMPLE, IN YOUR
UTILITY COMPANY; IF THEY HAVE ONE OR TWO CRITICAL PEOPLE
WHO ARE NECESSARY TO REPAIR UTILITIES, IF THEY ARE NOT
FUNCTIONING TO WORK ON THE SYSTEM THAT THEY HAVE TRAINED SOME
BACKUP INDIVIDUALS SO THAT THINGS CONTINUE.
WHAT DOES IT MEAN ON THE GOVERNMENT SECTOR?
VERY MUCH THE SAME THING.
MAKE SURE, AND WE NEED TO MAKE SURE THAT PLANS ARE IN PLACE
SO THAT THE FUNCTIONS OF GOVERNMENT, PARTICULARLY
PUBLIC SAFETY AND PUBLIC HEALTH, CONTINUE; AND AS A FORMER ATTORNEY
GENERAL AND GOVERNOR, ONE THING I WOULD SAY TO THE STATE AND LOCAL
OFFICIALS HERE IS MAKE SURE YOUR PAYROLL CONTINUES.

GOT A BACKUP.
WE HAVE TO REACH OUT IN EACH AREA OF THE COUNTRY TO STATE

LOCAL AND STATE LEVEL.
I TALKED ABOUT BUSINESS AND THE PRIVATE SECTOR BUT THEY'RE GOING
TO BE AFFECTED IN OTHER WAYS AS WELL.

ONE OF THE LESSONS WE LEARNED THIS SPRING WAS HOW CRITICAL SCHOOLS

CHILDREN ARE DISPROPORTIONATELY AFFECTED AND SO WORKING THROUGH
THE GUIDANCE FOR SCHOOL DISTRICTS IS NOT EASY.
YOU WOULD THINK IT WOULD BE EASY.
IT IS NOT EASY NOR IS IT ALWAYS UNIFORM, BUT WHEN YOU CLOSE
A SCHOOL, THERE ARE RIPPLE AFFECTS ALL ACROSS A COMMUNITY BECAUSE
THEN CHILDREN HAVE TO STAY AT HOME.
WELL, WHAT DOES THAT MEAN FOR WORKING PARENTS?
THAT MEANS THEY PROBABLY, AT LEAST ONE OF THEM, HAS TO STAY AT HOME
AS WELL SO, EVEN THOUGH THEY MAY NOT HAVE THE FLU, THEY'RE GOING
TO CONTRIBUTE TO THE ABSENTEEISM THAT INTERFERES WITH THE
CONTINUATION OF BUSINESS, OF GOVERNMENT AND THE LIKE
SO WE NEED TO THINK AND WORK THAT PROBLEM THROUGH AS WELL.
KIDS AT HOME: YOU'VE CLOSE THE SCHOOL; WELL, WE HAVE A LOT
OF CHILDREN IN THIS COUNTRY WHO GET BREAKFAST AND LUNCH AT SCHOOL
AND IS PROVIDED FOR BY THE DEPARTMENT OF AGRICULTURE,
AS YOU KNOW, THROUGH THE FREE LUNCH PROGRAM.
WE NEED TO BE WORKING THE PROBLEM: HOW ARE THEY GOING TO GET FED?
HOW IS FOOD GOING TO BE MADE AVAILABLE IF
A SCHOOL HAS TO CLOSE?
SO THESE ARE VERY PRACTICAL ISSUES.

WE HAVE INDIVIDUALS, PARTICULARLY IN THE FEDERAL GOVERNMENT, WHO
WORK WITH THE PUBLIC ALL THE TIME.
TSA INDIVIDUALS WHO ARE OFFICERS WHO ARE AT OUR AIRPORTS: PEOPLE
IN THE BORDER PATROL AND CUSTOMS WHO ARE POLICING OUR BORDERS.

THE ILLNESS ISSUES THEY MAY HAVE, THERE ARE ISSUES ABOUT PERSONAL
PROTECTIVE EQUIPMENT AND HOW THAT IS SUPPLIED TO THEM.
WHAT KIND OF MASKS SHOULD THEY HAVE?
WHERE ARE THEY?
HAVE WE STOCKPILED THEM?
ARE THEY IN THE RIGHT PLACE?
DO WE KNOW HOW TO GET IT TO EMPLOYEES WHO ARE IN THAT
TYPE OF SITUATION?

ABOUT, IN PARTICULAR, YOUR PUBLIC SAFETY COMMUNITY AND AT WHAT POINT

IT WILL ALSO AFFECT YOUR GUARDS AND YOUR CORRECTION OFFICERS,
SO YOU HAVE TO BE THINKING THROUGH WHAT DO YOU DO WITH
THE INSTITUTIONS THAT YOU HAVE WHO HAVE INCARCERATED POPULATIONS?
AGAIN, THESE ARE PROBLEMS WE NEED TO WORK THROUGH.
THAT'S WHY WE'RE HAVING THIS SUMMIT, TRYING TO EVOKE IN
ALL OF US A LOT OF THE ISSUES SO WE BEGIN OR REFINE THE PLANS THAT
WE ALREADY HAVE MADE.

BEEN A GREAT, GREAT PARTNERSHIP.
WE ALL CAME INTO THIS IN A HUGE LEARNING CURVE [UNINTELLIGIBLE]
PARTNERSHIP AND THE SENSE OF COLLABORATION HAS BEEN FANTASTIC.
I HAVE VIEWED THIS, FIRST AND FOREMOST, THROUGH THE LENSE
A PARENT. I HAVE TWO YOUNG CHILDREN AT HOME.
I THINK LIKE ALL PARENTS I WANT TWO THINGS.
I WANT MY CHILDREN TO BE SAFE.
I WANT TO KEEP THEM LEARNING SO AS OUR DEPARTMENT THINKS THROUGH WHAT
WE DO GOING FORWARD, THOSE TWO THINGS, KEEPING CHILDREN SAFE
AND KEEPING THEM LEARNING ARE ALWAYS GOING TO FRAME
OUR CONVERSATION.
AND THE MOST IMPORTANT THING WE CAN DO NOW IS TO THINK AND PLAN.
I WANT TO THANK, AGAIN, REITERATE ALL OF OUR THANKS FOR YOU TAKING
THE TIME NOW TO TAKE STOCK, TO SEE WHERE WE'RE AT,
TO THINK ABOUT THE NEXT STEPS.
AS YOU KNOW, WE HAVE SCHOOLS AROUND THE COUNTRY OPENING
OVER THE NEXT TWO MONTHS AND THIS IS THE PERFECT OPPORTUNITY FOR US
TO COME TOGETHER AND REALLY THINK ABOUT HOW WE HANDLE THAT.
I'M PLEASED TO REPORT THAT MOST SCHOOL DISTRICTS AND SCHOOLS
HAVE DEVELOPED GOOD, EMERGENCY MANAGEMENT PLANS.
MANY OF THESE PLANS ADDRESS THE FLU OR OTHER CONTAGIOUS DISEASES.
HOWEVER, SCHOOL DISTRICTS -- WE ARE CONSTANTLY ENCOURAGING
THEM TO REVIEW AND UPDATE THEIR EMERGENCY PLANS AND ENSURE THEY
ADDRESS BOTH PRESENT AS WELL AS EMERGING NEEDS AND OVER THE PAST
COUPLE YEARS WE'VE PUT OUT OVER $200 MILLION IN GRANTS TO OVER
700 SCHOOL DISTRICTS TO ENABLE THEM TO IMPROVE AND TO EXPAND
UPON THEIR EMERGENCY MANAGEMENT PLANS.
WE ABSOLUTELY REALIZE IN A CRISIS LIKE THIS WE CAN'T BEGIN TO MANAGE
IT BY OURSELVES.
THE LEAD HAS TO COME FROM THE SCIENTISTS, FROM THE DOCTORS
AND SO THIS IDEA OF PARTNERSHIP AND COLLABORATION: THIS THING
YOU HEAR REPEATEDLY IS SO CRITICALLY IMPORTANT TO US.
WE'VE TRY TO WORK VERY, VERY HARD TO DEVELOP STRONG PARTNERSHIPS
WITH CDC, HHS, DHS, THE DEPARTMENT OF AGRICULTURE, AS WELL AS A HOST
OF COMPONENT OFFICES WITHIN THOSE AGENCIES AND WE STRONGLY ENCOURAGE
ALL SCHOOLS TO DEVELOP SIMILAR PARTNERSHIPS AT THE STATE AND
THE LOCAL LEVEL.
THAT IS CRITICALLY IMPORTANT.
PARTNERSHIPS WITH THEIR PUBLIC HEALTH AGENCIES, MENTAL HEALTH
AGENCIES, EMERGENCY MANAGEMENT AGENCIES, FAITH-BASED
ORGANIZATIONS, YOUTH SERVING ORGANIZATIONS AND THE BUSINESS
COMMUNITIES ARE CRITICAL FOR US HANDLING THIS IN A THOUGHTFUL WAY.
WE WANT TO TRY AS MUCH AS WE CAN TO KEEP SCHOOLS OPEN.
WE RECOGNIZE THAT SCHOOLS MAY HAVE TO CLOSE.
AT ITS PEAK IN MAY, WE HAD 726 SCHOOLS CLOSE AROUND THE COUNTRY.
THAT IMPACTED ALMOST 500,000 CHILDREN.
BUT GOING FORWARD, IF WE HAVE TO GO THAT ROUTE, WHETHER IT'S FOR
A DAY OR A WEEK OR FOR A MONTH, AGAIN, IT'S CRITICALLY IMPORTANT
TO ME THAT LEARNING CONTINUES.
SCHOOLS MUST DEVELOP PLANS FOR PROVIDING ALL STUDENTS WITH
EDUCATIONAL SERVICES DURING ANY SHUTDOWN.
AND THAT DECISION TO CLOSE SCHOOLS IS SOMETHING THAT SHOULD NEVER
HAPPEN AT THE FEDERAL LEVEL.
THAT'S NOT SOMETHING THAT WE'RE EVER GOING TO DO.
THOSE TOUGH DECISIONS, THOSE, YOU KNOW, FINE LINES THAT WE
HAVE TO WALK HAVE TO BE MADE AT THE STATE AND THE LOCAL LEVEL,
AND WE ARE COMMITTED TO DOING EVERYTHING WE CAN TO SUPPORT
THE DECISIONS BEING MADE AT THAT STATE AND LOCAL LEVEL.
FINALLY, WE DON'T WANT ANY BUREAUCRATIC ISSUES TO GET
IN THE WAY OF DOING THE RIGHT THING BY CHILDREN, SO FEDERAL
EDUCATION CLIMATES WILL NOT BE AN IMPEDIMENT TO CLOSING SCHOOLS IF
CLOSING SCHOOLS IS WARRANTED AND RECOMMENDED TO ENSURE THE HEALTH
AND WELFARE AND WELL-BEING OF OUR STUDENTS, AND WE'LL DO EVERYTHING
WE CAN TO GIVE CAREFUL AND QUICK CONSIDERATION TO WAIVE A REQUEST.
FINALLY, I WANT TO THANK -- WE'VE HAD A SMALL TEAM THAT IS WORKING
THIS ISSUE ON A DAILY BASIS WITHIN THE DEPARTMENT OF EDUCATION.
THEY PROVIDED PHENOMENAL LEADERSHIP AND I WANT

THE OTHER QUESTION WE HAD WAS IN THE GUIDELINES THAT WE HAD BEFORE
THERE WAS THAT WORD "DISTRICT" AND A DISTRICT, AS YOU KNOW,
BEING A CEO OF A BIG CITY SCHOOL DISTRICT AND WE'RE BLESSED TO
HAVE SOMEONE WITH THAT PRACTICAL HANDS-ON BACKGROUND; A DISTRICT
FOR CHICAGO IS MUCH DIFFERENT THAN A DISTRICT FOR A RURAL AREA WHERE
YOU HAVE MAYBE THREE ELEMENTARY FEEDERS, A MIDDLE SCHOOL AND
ONE HIGH SCHOOL.
HAVE WE BEEN TAKING A LOOK AT THAT TO TIGHTEN THAT FOR THE FUTURE?
[ARNE DUNCAN] YEAH, ABSOLUTELY AND, AS YOU SAID, A DISTRICT CAN MEAN
ONE SCHOOL OR IT CAN MEAN 1300 SCHOOLS OR 1400 SCHOOLS IN

IN REALLY LOOKING AT IT ON A SCHOOL BY SCHOOL, COMMUNITY
BY COMMUNITY BASIS AT THE LOCAL LEVEL IS THE ONLY
WAY TO ATTACK THIS.

I WISH I COULD ANSWER THAT QUESTION WITH A DEFINITIVE
RESPONSE, BUT THE TRUTH IS, WE DON'T KNOW AND WHAT EVERYBODY,
ALL THE EXPERTS TELL US, IS PREPARE TO BE SURPRISED AT
EVERY STEP ALONG THE WAY.
SO WHAT WE KNOW IS THAT 1918 LOOKED VERY DIFFERENT THAN
1976 AND THAT LOOK DIFFERENT THAN THE LATE 60S AND WE DON'T KNOW
EXACTLY WHAT WILL HAPPEN WITH THIS VIRUS STRAIN, WHEN AND
IF IT MUTATES WITH THE SEASONAL FLU.
WHAT WE KNOW IS THAT THERE IS WIDESPREAD TRANSMISSION.
WE KNOW WHO IS BEING AFFECTED RIGHT NOW.
WE ARE WATCHING VERY CLOSELY, HAVE SIGNIFICANT NUMBERS OF
SURVEILLANCE PERSONNEL IN THE SOUTHERN HEMISPHERE RIGHT NOW
BECAUSE THEY ARE IN SEASONAL FLU SEASON AND HAVE H1N1 AND
NO VACCINE SO WE CAN WATCH WHAT OCCURS ON AN UP CLOSE
AND PERSONAL BASIS.
WHAT, AGAIN, MARK, WE'VE BEEN ADVISED TO DO, AND I THINK
THAT'S WHAT THIS SUMMIT IS ALL ABOUT, IS TO ANTICIPATE A VERY
SERIOUS OUTBREAK: KIND OF THE WORST-CASE SCENARIO AND LOOK
AT ALL THE CAPACITY AND SURGE CAPACITY AND PREPAREDNESS
THAT WE CAN, KNOWING THAT IT'S A LOT EASIER TO WALK BACK AS
WE LEARN MORE.
IF WE LEARN THAT THE FLU IS NOT AS SEVERE AS FEARED, IF WE LEARN THAT
IT DOESN'T -- IT GOES AWAY, POTENTIALLY, SOMETIME THIS
SUMMER OR FALL AND DOESN'T REAPPEAR WE CAN STEP BACK
FROM OUR PLANNING.
WHAT WE CAN'T DO IS WAIT UNTIL OCTOBER AND THEN SUDDENLY DECIDE
THAT WE HAVE A VERY SERIOUS SITUATION ON OUR HANDS SO
WE NEED TO BE VERY AGGRESSIVE ABOUT PREPARATION AND PLANNING,
ASSUMING THAT IT WILL COME BACK IN A MUCH MORE SEVERE FORM,
AND WE NEED TO BE PREPARED WITH BOTH A MASS VACCINATION PROGRAM
AND, AS SECRETARY NAPOLITANO HAS ALREADY OUTLINED, WITH A LOT OF
CONTINGENCY PLANNING FOR BUSINESSES AND SCHOOLS
AND FOR A VERY SERIOUS OUTBREAK,

I'D LIKE TO ASK SECRETARY SEBELIUS ABOUT THE PROSPECT OF A MASS
VACCINATION EFFORT THIS YEAR.
WE DISTRIBUTED THROUGHOUT VARIOUS PARTS OF OUR STATE SOME OF THE
ANTIVIRAL MEDICATION THAT WAS RELEASED FROM THE NATIONAL
STOCKPILE BUT MY QUESTIONS ARE: WILL THERE BE ENOUGH DOSES?
HOW MIGHT WE ORGANIZE IF YOU'RE GOING TO RECOMMEND UNIVERSAL
VACCINATION, SUCH A PROCESS, AND, PERHAPS, IMPORTANTLY, WHO'S GOING
TO PAY FOR IT?
[KATHLEEN SEBELIUS] [LAUGHTER] WELL, ALL GOOD QUESTIONS JIM.
THE GOOD NEWS IS THAT THE CURRENT EVIDENCE IS THAT THE ANTIVIRALS
ARE EFFECTIVE IN TREATING THE FLU.
BOTH TAMIFLU AND RELENZA SEEM TO BE EFFECTIVE.
THAT COULD CHANGE BUT CURRENTLY WE DO HAVE EFFECTIVE ANTIVIRALS
SO HAVING THE STOCKPILES PUSHED OUT TO HEALTH DEPARTMENTS
THROUGHOUT STATES, I THINK, IS A CRITICAL PIECE OF THE PUZZLE.
THE VACCINATION EFFORT IS CURRENTLY UNDER WAY:
GROWING THE FLU STRAIN TO SEE IF WE HAVE AN EFFECTIVE VIRUS TO
IMMUNIZE FOLKS AGAINST H1N1 AND, AT THE SAME TIME, TESTING THE
SAFETY AND EFFICACY TO FIGURE OUT HOW MANY DOSES WILL BE REQUIRED,
AT WHAT LEVEL, MAKING SURE THAT THE SIDE EFFECTS ARE NOT WORSE
THAN THE VIRUS ITSELF; SO SAFE AND EFFECTIVE VIRUS, SAFE AND
EFFECTIVE VACCINE, WE THINK, WILL BE READY, IF EVERYTHING
GOES RIGHT, BY MID-OCTOBER.
THE TARGET POPULATION RIGHT NOW LOOKS LIKE IT'S YOUNGER AMERICANS
AND OLDER FOLKS WHO HAVE UNDERLYING HEALTH CONDITIONS.
SO WE WANT STATES TO BEGIN TO PRIORITIZE BASED ON THAT,
BECAUSE ALL THE VACCINE WON'T BE AVAILABLE SIMULTANEOUSLY.
IT WILL BEGIN TO BE AVAILABLE IN MID-OCTOBER BUT THEN WE'LL BE --
THERE'LL BE A PRODUCTION TIMETABLE SO PRIORITIZING WITHIN STATES AND
FIGURING OUT IF IT REALLY IS A SCHOOL AGE POPULATION AND A DAY
CARE POPULATION: WHAT THE BEST POSSIBLE SITES ARE
TO GET TO THAT POPULATION.
YOU'LL BE RELIEVED TO KNOW THAT THE PRESIDENT AND CONGRESS HAVE
BEEN WORKING ACTIVELY ON THIS PLAN AND FEEL VERY STRONGLY THAT SAFETY
AND SECURITY OF THE AMERICAN PEOPLE IS THE NUMBER ONE
PRIORITY, AND AT THIS POINT, CONGRESS HAS APPROPRIATED
FUNDS TO BEGIN THE DISTRIBUTION PROCESS.
WE'VE ALREADY APPROPRIATED ABOUT A BILLION DOLLARS
TO BUY THE BULK INGREDIENTS.
THERE IS ANOTHER SEVEN AND A HALF BILLION DOLLARS, SOME OF IT IN
PREPAREDNESS FUNDS, SOME OF IT IN OTHER FUNDS THAT WILL BE AVAILABLE
AND THIS WILL BE A FEDERALLY FUNDED VACCINE PROGRAM THAT
MAY HAVE AN OPPORTUNITY TO RECOVER SOME FUNDING BACK FROM INSURED
FOLKS WITH PRIVATE INSURANCE BUT RIGHT NOW WE DON'T ANTICIPATE,
GIVEN THE TIMETABLE AND THE RAPID NEED TO GET THIS OUT, THAT THIS
WILL BE DONE THROUGH PRIVATE SOURCES THE
WAY THE SEASONAL FLU IS.
THIS WILL BE REALLY A PUBLIC EFFORT, FUNDED
BY THE FEDERAL GOVERNMENT.

FEDERAL GOVERNMENT IS NOT GOING TO GET IN THE WAY AND WE WANT YOU TO
MAKE THE LOCAL DECISIONS AND CLOSING SCHOOLS, OBVIOUSLY,
IS ONE OF THOSE THINGS THAT IS A LOCAL DECISION: SHOULD BE MADE
AT THE LOCAL LEVEL.
WHAT WE FOUND DURING THESE PAST MONTHS, HOWEVER, IS THE BIG
QUESTIONS SEEMED TO COME AFTER THAT DECISION IS MADE BY THE
SUPERINTENDENT OR THE SCHOOL BOARD OR WHAT
EVER; SO A COUPLE OF THINGS.
YOU KNOW, WOULD THE FUNDS, IF THERE IS ANY QUESTION ON THE
FEDERAL LEVEL, FEDERAL DOLLARS, CONTINUE IF,
IN FACT, THE ATTENDANCE COUNTS DROP OFF AND THEY DON'T MEET,
WHETHER IT'S FEDERAL REQUIREMENT OR WHATEVER, FOR RECEIVING
FEDERAL FUNDS AND IT'S REALLY A THREE-PART QUESTION HERE.
IF THERE IS A WAIVER REQUIRED; FOR EXAMPLE, UNDER THE NO CHILD LEFT
BEHIND, WILL YOU GRANT THAT WAIVER RELATIVELY QUICKLY IF, IN FACT,
IT BECOMES A PROBLEM?
AND THEN THE LAST THING; IT REALLY PROBABLY IS A STATE-BY-STATE ISSUE
BUT WE HAVE STATUTES THAT REQUIRE A NUMBER OF DAYS TO BE IN SCHOOL
AND WE HAD A LOCAL ISSUE, YOU KNOW, AND WE HAVE SNOW DAYS.
WE HAVE OTHER THINGS THAT HAPPEN WHERE SCHOOLS ARE CLOSED AND THEN
ALL OF A SUDDEN KIDS ARE GOING INTO JULY AND THEY'RE SAYING
CAN YOU JUST WAVE THIS REQUIREMENT FOR US?
SO WE'RE LOOKING TO YOU FOR ANSWERS ON THE FEDERAL LEVEL,
YOU KNOW, AND SORT OF AN ASSURANCE THAT WE CAN GET THOSE WAIVERS IF
NECESSARY, AND FRANKLY, FUNDS IF NECESSARY.
